You will directly coordinate the patient's discharge including supplies, transportation, and other services to ensure a safe transition. Your day-to-day

• Research, identify, and arrange external services for residents such as Chaplaincy, Advocates, Responsible Parties, and optimal care environments.

• Directs, timely and efficient, five-day discharge planning meetings for all newly admitted residents.
• Assesses patient's support systems and home situation to assist in decision-making regarding discharge plan and needs.

• Meet with and assist residents with social issues such as conflict resolution, problem-solving, and care coordination.
• Conduct customer satisfaction surveys.

The typical schedule for the MDS Coordinator is Monday-Friday with a weekend rotation of Manager on Duty.
